Insurances: We accept Cigna, Optum Health, Tricare, MHN/Health Net and Out of Network. For out of network, client pays full rate of the session and we submit billing to the insurance.
In order to verify your insurance coverage, the following information is needed:
Client's name and date of birth
The primary subscriber's name as it appears on the insurance card, date of birth, phone number, address and relationship to the client
Insurance Company and phone number
Subscriber ID#
Client’s phone number where you can be reached if any questions arise
Questions to ask your Insurance Provider (out-of-network):
STEP 1: Find out what percentage is covered for “out-of network” or “non-participating providers.”
STEP 2: Find out the “maximum allowed amount” per session for zip code 90247 for an LCSW. They may ask for procedure code such as 90837 which is 60 minutes therapy session. Often insurance reps aren’t well trained and don’t know what the maximum allowable amount is so be sure to ask for a supervisor if they can’t help you.
STEP 3: Find out if you have a deductible for out of network providers and how much it is.

Your appointment time is reserved just for you. There is a 24 hour cancellation policy. Otherwise, you will be subject to pay full agreed upon rate for your missed appointment. Illness and emergencies are dealt with on an individual basis with your therapist. Insurance plans will not reimburse a missed appointment.
